Two Health Plan Levels
The difference among the two plans is the Level of Reimbursement. The level is determined by the co-pay, deductible or co-insurance, and the maximum out of pocket expense not including the deductible. Click on each plan level below to view the Summary of Benefits and Coverage.
- ULTIMATE - Highest Level of Reimbursement ($30 co-pay, no deductible, no co-insurance)
- BEST - Lesser Level of Reimbursement ($30 co-pay, $250 deductible & 20% co-insurance)
Both plans offer:
- Identical Medical, Prescription, and Behavioral Health covered services with identical monthly premiums.
- Choice to utilize the Network or Non-Network Providers.
- Choice to utilize Wellbeing Programs.
- Choice to utilize Teladoc.
2025 Medical Plan Rates
The premium deduction applies to 24 pay periods in a year.
Employee Only: $46.15 per pay period ($92.30/month)
Employee + Spouse: $173.87 per pay period ($347.74/month)
Employee + Child(ren): $149.03 per pay period ($298.06/month)
Employee + Family (Spouse & Child[ren]): $210.94 per pay period ($421.88/month)
Dependent Child: Age 26-30: $408.38 per pay period ($816.76/month)
