You can prevent diabetes!

You have the opportunity now to prevent Type 2 Diabetes. In fact, you have a 66% chance of NOT developing diabetes by becoming a bit more active, eating healthier and losing some weight. We know that making healthy choices can be difficult, so we’d like to support you in these efforts.

Eligibility Criteria:
  • Age 18+
  • BMI of 25.0 or greater (or 23+ if Asian descent)
  • AND one of the following:
    • BMI of 27 or greater
    • Hypertension
    • Prediabetes
    • Tobacco Use
    • Dyslipidemia (high cholesterol)

NOTE: Those who have been diagnosed with diabetes are not eligible to particiapte in this program. Please contact Florey Miller for information about available resources, including an Omada program tailored to meet the needs of those already living with diabetes.

There is no cost for you to engage in this program - this is offered to you as a member of the Health Plan.

*Some exclusions apply. Those who have diabetes, are pregnant, have an eating disorder, had bariatric surgery in past 6 months, receiving dialysis treatment, or have other medical or psychiatric conditions that prevent them from participating, are ineligible.

Meet Your Diabetes Educator

Florey Miller, MS, RD, CDE

Registered Dietitian / Diabetes Educator

Florey is a Registered Dietitian/Nutritionist and has been a Certified Diabetes Educator since 1999. Her background includes working as clinical dietitian for Blake Medical Center, as diabetes educator at Doctors Hospital and she had a private practice in a cooking school.
