- Age 18+
- BMI of 25.0 or greater (or 23+ if Asian descent)
- AND one of the following:
- BMI of 27 or greater
- Tobacco Use
- Dyslipidemia (high cholesterol)
NOTE: Those who have been diagnosed with diabetes are not eligible to particiapte in this program. Please contact Florey Miller for information about available resources, including an Omada program tailored to meet the needs of those already living with diabetes.
There is no cost for you to engage in this program - this is offered to you as a member of the Health Plan.
